Here you will find all the relevant information to apply for a scholarship from our foundation.

INSTRUCTIONS

Here are the instructions and important dates for applying for and participating in the Dr. Carlos López Somolinos Foundation scholarship selection process.

Please verify all required documentation and pay attention to the document submission deadlines.

Important Dates:

Applications will be sent to schools, foster homes, and interested individuals during January 2026.

Application and Document Submission Period:

The application period begins on January 12 and ends on March 31, 2025, at 11:59 p.m.

Note: To be considered, your application must be complete, with all questions answered and all required documentation submitted.

The only document that should remain pending is a copy of the 2025 income tax return, if it has not been filed by April 15, 2026.

All documents must be received by April 18, 2025.

Selection and Final Evaluation Period:

This period will run from April 1 to April 25, 2025.

After a preliminary selection, potential scholarship recipients may be contacted for a final evaluation and/or interview.

Final Selection Notification:

The final selection notification will be sent by mail

(Please include the mailing address as required by the USPS) after final approval by the Foundation’s Board of Directors during May 2026.

REQUIRED DOCUMENTS

Basic Student and Family Information

  • Complete Application: Student Information, Father’s Information, Mother’s Information, Family Environment Report, Student Questionnaire (please do not leave blank spaces; write N/A if applicable)

School Report

  • Copy of grade report – must have a GPA of 3.0 or higher.
  • School Report – included with the application (must be completed by one of the candidate’s teachers or the school principal and preferably sent directly from the school to maintain confidentiality).
  • Two letters of recommendation, preferably from two of the candidate’s teachers.

Financial Statement

  • Financial Statement – ​​document included with the application – Please answer all questions or write N/A if applicable.
  • Copies of tax returns filed with the Puerto Rico Department of the Treasury for both parents, or jointly if filing as a couple *
  • Copies of W-2 forms for both parents or guardian * (*Please shade or cross out the Social Security numbers on both documents)
  • Debt certificates or payment receipts from the Child Support Administration (ASUME) (both parents)
  • Certificate of assistance from the Department of the Family and information on any other assistance received, such as from the Department of Housing or Social Security, disability benefits, etc.
  • Certificate from the Nutritional Assistance Program (PAN)
  • Proof of physical address (envelope or letter with physical address)
  • Any other information that may help demonstrate social or economic need and/or highlight the qualities of the scholarship applicant.
